Hospice Sunset
Licensed in 2007, Hospice Sunset is one of the newest programs of Lutheran Sunset Ministries, whose mission is to provide end-of-life services to the residents of Bosque County. Hospice services provide support to the family or caregiver, as well as the patient. Hospice services help patients and families manage the physical, emotional, social, financial and spiritual aspects of a terminal illness.
An interdisciplinary team works with the physician and the caregiver to manage the symptoms of the terminal illness, and provide care and comfort. These services are provided by team members in a patient’s home whether “home” is in residence, an assisted living facility, a nursing facility, or a hospital.
The goal of the team is to help patients live the last part of life with dignity, comfort, and peace while also caring for the needs of loved ones. The team includes
- Physicians
- Nurses
- Social Workers
- Home Health Aides
- Counseling Services/Chaplains
- Trained Volunteers
Hospice also arranges for therapies, medical equipment, supplies, and medication related to the terminal diagnosis/illness. In addition to visits by the hospice team, trained volunteers offer a multitude of services and support, enhancing the quality of life for the patient and loved ones.
An integral service of Hospice Sunset is spiritual counseling and support. These services are provided by the chaplain who is available for the patient, as well as for the family and friends. The chaplain may also coordinate services with the patient’s individual faith organization for additional support. Bereavement and counseling services are available throughout and following the death of the patient.
Hospice Sunset is located off campus at 215 West 5th Street in Clifton at 254-675-3391.
